(2) fill in the form (all columns are to be filled in, like HP, emails, address, next of kin)
(3) save it, with file name AVIVA CIP 2011_Your Class_Your Name
(4) send it to hendersonsecondary@gmail.com
(5) only for 100 pupils (Sec 3/4/5)
(6) No group registration is allowed
(7) Pupils with discipline issues will be assigned to be RESERVES for the CIP
